I consent and agree to take the company required pre- and/or post-employment drug screen at the medical facility or laboratory designated by Bridgewater Interiors. I understand and agree that a specimen will be tested for the presence of drugs or medication in my body. I further understand that I may be requested to take a physical/medical examination following an offer of employment, and my continued employment will be conditioned upon my satisfactorily completing such physical/medical exam. I further consent and agree to the release of the laboratory results of any tests performed on the specimens to Bridgewater Interiors. I will hold no person, corporation, medical facility or organization liable for any damage which may result from my participation in the drug screen process.

I recognize that neither this application, nor any future employment is an employment contract. I recognize that if I become employed, my employment will be at-will and for no definite period of time. I will be free to terminate my employment at any time for any reason, and the Company retains the same right. No company representative has the authority to make any contrary agreements.

I understand that this application will be considered active for six months. After that, if I have not been employed by the Company, I must reapply if I still wish to be considered for future employment.

I certify that the information in this application is correct to the best of my knowledge and I understand that deliberate falsification of this information is grounds for refusal to hire or dismissal from employment.

I understand it is the policy of Bridgewater Interiors to check references offered by applicants and that it is their objective to obtain information on ability and previous job performance for the sole purpose of considering me for employment. I hereby authorize any person named by myself as a reference, prior employer, or an educational institution to submit information or opinions about me in order to evaluate my employment qualifications.
